Fill in a Spreadsheet with Data

We are in need of a freelancer or team whom can fill in data in a custom made form.
The individual or team would need to input data for zip codes (already in the custom made form) and then fill in data into two spots (for local senator and assembly man associated with that zip code)
However, the individual or team would have to look up the local senator and assembly man associated with each zip code on a separate site. (link provided)
All the local zip codes, senators, and assembly men have already been imputed into the custom made form as options.
Each zip code has a "page" so to speak, created in the form with a section for the assembly member and senator (which can be chosen by entering in which district the assembly member or senator is associated with)
It is really quite simple to do; however, we are on a time schedule and need the work completed A.S.A.P.
