Build us a one-page Chinese website

We have written a short e-book to help smokers in China quit. Before launching it into the wider market, we want to test its popularity and effectiveness with a sample of 1,000 Chinese smokers.
To recruit these smokers, we want to create a one-page website which explains a little about the book, and allows smokers interested in quitting to input their email address to receive a free copy of it.
At this point this is a research project, so we need to ask potential readers a few basic questions before we send them the link to the e-book (age, level of education, how much they smoke, brand smoked etc.) and of course we also want their feedback after they read it (Did they quit after reading it? Did they think it was helpful? How can we improve it? Would they recommend it to a friend?).
If the research shows this approach to be effective, then we will look to launch it to the wider market in China at which point we will invest more heavily in a site including video and other content.
