File Renamer script to rename all video files (mp4,mkv..) on a folder

I need a script that will rename all my video files on a specified folder.
The files is usually in mp4, mkv format.
I want to rename all files on that folder to this format:
Name of file (i will write on script) + Number (script will organize it with starting from 001) + extra prefix (i will write on the script too)
Example of the format:
Video Name [login to view URL]
"_LEG" is the extra prefix
