Script to attain email address.

I need a script which will be placed on a webserver, that does 2 things:
1. Script captures the email address of a person visiting the website (if they visit from an email, such as gmail, yahoo, outlook, etc...)
2. Script outputs this email address onto mysql database, along with the time & date.
