We have the attached pdf form that we would like to have coded so that once the stage complete section is ticked the users name, date and time are automatically populated in the corresponding box, these will be completed in stages so as the next stage is completed and ticked the person completing that sections credentials will populate. We don't want the file size to become too big whilst doing this.
