I need a custom form created for my WooCommerce WordPress website. The form is to be multi-page and conditional. The ‘Next’ button on the last page of the form is to add a product to the cart and then take the customer to the checkout page. There is to be a progress bar along the top of the form. There is to be a 'Previous' button on the bottom left of the form on each page, except the first page. There is to be a 'Next' button on the bottom right of the form on each page. The form is to be styled with CSS.

Form Question #1:
The three radio choices are to be displayed as images.
Example:
What type of fragrances do you prefer?*
Unisex (Value=1), Male (Value=2), Female (Value=3)
ANSWER: Female (Value=3)
Form Question #2:
This question is on a new page and the customer is taken here upon clicking the 'Next' button on the first question. Four pictures are shown and the customer is asked to rearrange them from best to worst by dragging them with right being best and left being worst. The pictures are arranged in a row with a little space between them.
Example:
Reorder the scents from your favorite (right) to your least favorite (left).*
Floral Pic (Value=1), Fresh Pic (Value=2), Woody Pic (Value=3), Oriental Pic (Value=4)
ANSWER: Oriental Pic (Value=4), Fresh Pic (Value=2), Woody Pic (Value=3), Floral Pic (Value=1)
Form Question #3:
This question is on a new page and the customer is taken here upon clicking the 'Next' button on the second question. This question is conditional and depends on Form Question #1. This is a check box question where multiple answers can be chosen. At least one choice must be selected. The four choices are to be displayed as images.
Example:

Which scents do you like?*
Fruity Pic (Value=1), Floral Pic (Value=2), Soft Floral Pic (Value=3), Floral Oriental Pic (Value=4)
ANSWER: Floral Pic (Value=2), Soft Floral Pic (Value=3), and Floral Oriental Pic (Value=4)
Form Question #4:
This question is on a new page and the customer is taken here upon clicking the 'Next' button on the third question. This question is on the same page as Form Question #4. It is to the left of Form Question #5 (see the example website). The two radio choices are to be displayed as images.
Example:
Are you a day person or a night person?*
Day (Value=1), Night (Value=2)
ANSWER: Day (Value=1)
Form Question #5:
This question is on the same page as Form Question #4. It is to the right of Form Question #4 (see the example website). The four radio choices are to be displayed as images.
Example:
Which season do you like most?*
Winter (Value=1), Spring (Value=2), Summer (Value=3), Fall (Value=4)
ANSWER: Summer (Value=3)
The ‘Next' button on the Form Question #5 page is to be conditional. It will add a product to the cart and then take the customer to the checkout page.
