Simple Booking System in php/mySQL with AJAX

I need a relatively simple php based booking system with admin backend. The system will allow for people to book a time slot within a day.
The backend will need to allow for manual adding of bookings as well; it will also need to allow admin to determine (based on location) how many timeslots needs to be allocated - for example somene booking a local appointemnt will only require 1 timeslot, but someone from further away 2 slots.
Frontend will need to be easy to use, intuitive and informative.
More details will be discussed with the chosen provider including a full feature list.
