i am looking for a active form ( or how you call it )
programming In Ajax, ( php )(no windows, asp, .net etc)
a form that i can extend with other fields ( name adres etc )

I am looking for a database
with all countries, states, cities and if posible zipcodes.
People select country
( if zip code is in then they add zip code and state ( adres ) shows)if not
then form shows all states of that country
when state selected it shows all cities.
when database support adresses it can show when zip is there the streets etc
when a state, countrie is not supported a selctox not in list and they can fill in manual. ( this must be easy changed in form so i can set those on hold and a administrator can check it and then release so it will be stored in database as aproved adding
so in short
a AJAX form that shows a list of countries
when selected country it shows states ( form that country)
afhter state it shows in pull down cities of that state
zip code optional and if in then it shows streets belong to that zip
option in form must be when a state, city is not supportat to select it and fill in manual
( must be posible to set it on hold for admin )
and to make it fast what is important only show states, cities belongng to selected
Most countries, states, cities must be in allready, so a up to date database
a explanation if needed how to setup database etc
