Contact Form Upgrade

I have a site already up, but my contact page is not working. It is a PHP and HTML issue. I need this done ASAP. I will send you the files and you agree to:

1) Fix the Submit button on the form

1.1) Make the form fields glow or highlight when clicked into them.

1.2) Make the Email field; verify it is an email format

2) Make a nice window (not a pop up window or pop up box) inside the page that says the form was submitted

3) Make a Captcha security section in the form - that we can easily turn off and on in the code

4) Keeping the same PHP / database structure that is currently built into the page

5) The received email from the form should be HTML based and not standard text

Final payment will be released when we test the form and assure it is working on our server. We will not give you access to our FTP.
