Add upload your own images to oscommerce

We are looking to get a series of pages designed for a heavily modified oscommerce shop. This new feature will allow customers to upload their own images wearing our products.
1 - customers will be able to browse andupload their own images to our server from an easy to use informative page (linked on the site and linked the [url removed, login to view] page)
2 - customers will be able to select a product from our products table to associate their uploaded images with (for each product one thumbnail and one larger image will be required.
3 - folder where images are uploaded will need to only allow for .gif, .jpg files to prevent attacks
4 - customers will be able to add a comment (up to a certain number of characters long) for each set of uploads.
5 - admin page will be created in oscommerce where staff can view all uploads/comments
6 - images will need to be approved by staff prior to publishing on the new page
7 - images will be linked to a customers account so that they can delete them if they choose.
8 - customers need to login in order to upload images
