I am looking for someone to create a fillable form for my company. The form will be PDF\'d and used as a report that is sent to my customers. I will be using the form myself, not my customers.

The form will be used to input data in the form of words and numbers and tables that I will be creating in Microsoft excel.

My business is to analyse utility (electricity specifically) bills and then provide a report to my customers based on my findings and my knowledge of rates and tariffs in south Africa.

I need the report that customers receive to look exceptionally professional and not appear to be a report that anyone could create using regular word-processing programs like Microsoft word and Microsoft excel.

I would provide the content/input fields required on the form as well as my company logo, artwork etc. which I already have.
