Data entry - transfer data from excel to htaccess file

Hello Freelancers,
I need some data to be transferred from excel files into htaccess file in specific way.
I have little more than 4000 rows in excel files like this:
[url removed, login to view] [url removed, login to view]
Each row have to be recorded in the htaccess file like this:
RewriteCond %{QUERY_STRING} ^page=term&id=52$ [NC]
RewriteRule ^[url removed, login to view]$ [url removed, login to view] [L,R=301,NC]
Pay attention that there are different terms, but the structure will be the same.
Need this to be done for 1 day. Thanks
