Amend a website and provide ongoing support

About us: We are a law firm that is taking the offline experience digital. We are partnering with strategic partners to provide their client's with the legal documents they require. We need someone to facilitate this!
We require a coder to facilitate our online legal program, this includes;
1. creating an online database for our clients, including online tracking of progress and payment systems;
2. create online forms that populate documents in our backend system (this includes coding formidable forms to populate data in our backend system)
3. create an online payment system;
4. basic design and optimisation of the website.
future work will include (and will be looked upon favourably);
1. ability to make website responsive and/or app based;
2. ability to identify potential issues and resolve them.
Ultimately, we are looking for a partner that will facilitate the progression of this website over a period of months/years.
We will require references and online examples of work that has been completed!
