Build a quiz for my Shopify store

1. What experience you need:
- Experience building custom pages on Shopify
- Experience building forms/quiz with answers shown
- Ability to inform us on the information you need to do this job BEFORE being awarded the project - please list what you need
- Links to shopify stores you have built/worked on as examples
2. What we can supply to you
- Quiz questions, answers
- Admin access to Shopify to build within
3. What the quiz is
- A quiz that tells people what product they should buy
- 1 question with multiple check boxes, where customers can select up to 5 areas they want to improve. The answer will be whichever product suits their needs
- Answer page that tells people which 1 or 2 products they should buy (if they select equal amounts of areas for 2 answers)
- Answer page will have an image of the product, description and link to buy now
