Microsoft Outlook Form

I would like a Form created (Is possible) My Company currently has Outlook as the Mail Client on Exchange of course. We have a Help Desk in which receive emails from users if they require assistance with there PC's or need additional access etc ...

The emails they send are not following certain guidlines and do not contain all necessary information. We need some consistency in the emails coming through. Can you create a form in outlook so that they must fill in certain fields before they can submit a helpdesk email. For example:

Asset information

Type of Problem

When problem occured.

Would it be possible that with a click of a button they can fill in the form and send it in proper format to the helpdesk. Will it have to be in a public folder or can each one of the users have a shortcut to the form within outlook and just click an icon and off they go ..?? ... Please let me know if anyone can do this ... Thanks
