Create form for file download but limit to single domain.

I need a form that works in conjunction with Opencart where buyers can purchase files that are locked to a single domain. Users will have to fill out their details as well as the domain the files will reside on before checking out and making payment. The form will insert an encrypted code into the files and zip them up for download after payment. The files will only work on the specified domain after uploading.
