OSCommerce - Standalone 1 Page order form (PHP) with corresponding admin page Description: Need 1 page order & admin forms that will insert orders & allow monthly summary report into an existing OsCommerce Mysql database. These are standalone pages and are NOT to be integrated with OsCommerce site. Only the database is the standard OsCommerce. See Deliverables section next to review all details:

Other details of this 1 page form: ** Instead of using the typical shopping cart of Oscommerce, I need 1 page order form where only 6 items & their prices will show. The buyer will check mark all items he/she needs. The order total should be computed. There is no tax or shipping charge. ** Two scroll boxes about 4 lines in height, containing warranty information and consent conditions (all text will be provided) with corresponding check box that forces buyer to agree, (buyer must check before clicking Submit) ** Billing and Shipping Address with capability to copy all Billing address to Shipping by a check box. These should be placed side by side and not one below the other. Sample screen shot will be provided. ** About 14 other informational Fields, about 20 characters in length (4 required and rest optional) that the buyer fills out. ** 1 comments field about 4 lines x 40 characters in size ** You can insert dummy text data in scroll boxes and descriptions of 14 fields. ** Credit card field with expiry, CVV etc. ** Your code MUST be easy to read with comments and descriptive variable names. You can expect an excellent feedback from me in return! Once the buyer clicks "Finalize Order", your PHP code should: 1. Insert data into OSCOmmerce tables: orders, orders_total, orders_status_history, orders_products. A "flag" field in orders table should be updated with number "22" to indicate that the order was entered from the stand alone page. 2. Generates email to the customer email address(s) (via mail function in php). 3. Data from the comments field and the 14 fields above should be added (collated) together into the comments field of the orders_status_history table. ** Corresponding required admin stand alone PHP page, not tied to OsCommerce should show a monthly running report. This page will simply show order number, customer name, amount, date/time of order with the total dollar amount for the month. A drop down month/year option should allow previous months summary. ** PLEASE provide link of previous work on OsCommerce. I would prefer to not provide access to the site and if you have your own OScommerce install where you can create , test this code, that would be great. However if you do not, it may be ok as I can take your code, load it and provide any errors back to you. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):

a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
