I need a single page form to submit automotive leads to an affiliate program. Instructions on the interface can be found at

I would like the form to be template based so I can change the design without disturbing your code. I expect a working form to be built with a simple interface that I can then later modify via templates.
I would also like to be able to upload the CSV file on a weekly basis and have the make/model selections updated.
You will need to test the form on their test server as described in the instructions and support initial setup on their live server.
My server is a Linux/Apache box with PHP4.
