80311 Custom pdf generated & emailed

Custom pdf emailed I need a program that will allow someone to enter there name, email address, and phone number from a website. After they enter there info in the web form it then needs to then be added into certain place in a pdf flyer and then emailed to that person. Also, there personal information needs to be captured and put in a online results page. Then flyer information will be the same for everyone and only there information will custom on the form.
