Create database (MYSQL) for Web

Database ONLY. NO DESIGN WORK. This job is for MYSQL coder.

Database must support the following functionality:

1. unique user ID (email address)

2. user registration (email address, first name, last name and password)

3. user login

4. user dashboard page (this page displays database tables/details) only.

Next, create an ADMIN LOGIN (web-based form) enabling us to view users and update user information as needed.

Create the following database tables and name them as follows; (the following information will appear on both user dashboard "after login" and "admin side"

/table name /type

1. minutes earned (numeric field)

2. visits remaining (numeric field)

3. appointment reminder (text field)

4. shade history with date (text + numeric)

5. date of last visit (data)

The admin form will enable us to update the above information by unique user ID. Example, as admin we can update # 1 minutes earned by inputting a numeric number and clicking save. This will then update on the user side dashboard.

USER DASHBOARD ( the following will be display only so user can log-in and view account [url removed, login to view] design, keep very basic; web page displaying the following information.

/table name /type

1. minutes earned (numeric field)

2. visits remaining (numeric field)

3. appointment reminder (text field)

4. shade history with date (text + numeric)

5. date of last visit (data)

Again, this project is extremely basic with no design needed; just create the database with tables and functionality.
