Simple App - Fill out PDF Forms for Company

I need a company app that will hold our forms and then an employee can open the app and complete a form and hit a submit button that emails the completed pdf to us. We want to be able to provide you pdf version forms and you make them available on our app to be filled out by our employees. We want this primary to be able to have our employees fill out time sheets and send to us.
Need an android and iPhone app.
