Create an android application

This project will be to convert an already existing web application to an android app. The web application currently captures data from customers in a tattoo shop and saves it to a database. The android app will communicate with a RESTFUL API which already exists. Attached are examples of what needs to be created on the android app. General steps of how the android should work:
1) The customer is presented with a form that they need to fill out. The form has required attributes that must be filled out. They must also sign their signature at the bottom of the form. Once the form has been filled out, they can submit it (page [login to view URL])
2) They are presented with a PDF version of their form with their filled out information (This PDF will be given to you). They have the option to go back and edit their info, or proceed (Page [login to view URL])
3) The tattoo artist then takes up to three pictures and uploads it to the form. Once all the uploads are finished they press "Complete Form" and the app goes back to step 1 (Step [login to view URL])
Bonus if you can create this application in IOS in the near future
