Remote Bluetooth Device

I have a device that has two leds as indicators of functionality. The white led indicates that it works properly and the red led show that in malfunctions. Now, I can’t be next to that device all the time, so I need an engineer that would create a simple as possible way/device that would allow me to monitor that particular device remotely.
I need a bluetooth device that I could connect to the leds power source and that would permanently show me on my phone/tablet No.1 what color/led is turned on at the moment and eventually to send me an instant notification when the led light is changing.
1. The leds power source is between 12 and 14 Volts and the medium distance between the bluetooth device and my phone/tablet No.1 is between 3 and 6 meters. My tablet/phone No.1 is always connected to the internet and is always in the same place.
2. At the same time what I need is that the app that is showing the leds status on my phone/tablet No.1 to be synchronized through the internet anytime I want with another device (phone/tablet/PC No.2) so that anyone who is granted access through an app/website to be able to check the led’s status.
