Custom Ticket Tab

We need someone to create a custom tab for our facebook page. You will be responsible for both design and development of the page. We will provide you with assets in order to complete the tab. Along with the design you will need to incorporate a form function on the page so we can collect information from the users. The promotion is a giveaway of concert tickets and the user must enter their name and email in order to win.
