Build a Website - Integrate Form on WordPress site.

I have a WordPress based website, and I need a sign up form integrated. When someone signs up they should be able to select their product and check out, through WooCommerce.

SO they will Select their Package, then enter all the information on the form I am asking you to integrate, and then proceed to checkout. The form which you are installing I would like a copy of it emailed to me every time someone signs up.

I need this done today.

Total budget is $50.00 USD will create a milestone for half and will not release any money until completion.
