Create PHP-to-PDF page using FPDF php extension

I want to create a page that will generate a PDF file using FPDF php extension.
I already have the script mostly build and working, I just need you to adjust it to fit the template, I can't figure out how to position the blocks of text correctly.
Please only bid if you have previous experience with FPDF and can start now.
