Add support of Maxmind's ISP and NETSPEED databases to the "ngx_http_geoip_module" module

Hello,

We need to modify the module "ngx_http_geoip_module" (([url removed, login to view])) for Nginx 1.4.6.

This module works with Maxmind's GeoIP database, but it supports only country, city and organisation databases.

We need to add support of Netspeed ([url removed, login to view]) and ISP ([url removed, login to view]) databases.

We use php5-fpm on our servers and the variables, with which the module works are defined in "fastcgi_params".

For example: fastcgi_param GEOIP_CITY $geoip_city; and in the php script then: getenv('GEOIP_CITY');

We need to add ISP and NETSPEED databases by the same way.

Databases are in the binary format in a .dat file.
