Order form required - no payment options
I require a form that will collect the usual contact details from a web visitor plus a model number, qty and description
However if the user selects a field that asks "do you want to order more models" the form is to open additional model number and description fields where the user can order more models with descriptions and quantities
Some fields will need to be validated with the visitor being returned to the form where a note next to an invalid entry will indicate that the field has not been completed correctly
The form results will be e-mailed to my client at his e-mail address in a neatly formatted manner so that he can print out the order form.
The form can be in PHP , asp or asp.net and the coder will be required to install and impliment the code onto my client's web site which is hosted on a windows iis server with asp, asp.net and PHP installed and configured.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
