Database planning and Form design for new Microsoft Access Database

Hello,
I have set up a very basic architecture for the first Microsoft Access database that I have ever attempted.
I have gotten as far as creating my tables and some of the relationships between tables. However, I still need help making sure that my database has been planned in a smart fashion, and I will need someone to set up the corresponding forms for data entry. So far I have 4 tables, the largest having 12 fields so far.
Anyone who is familiar with the petroleum industry or petroleum geology would receive special consideration.
This job would likely include the equivalent of a few hours of coaching (to get the design right) and support to get me up and running once the software portion of the project is complete.
In my best estimate, the job would be 2 hours of database design and planning work, however long it would take you to do the forms for said tables, and an additional 2 hours of support afterwards.
