Create an Advanced Search Page Using PHP/MYSQL for a dating site

The code is 99% finished. There is already a search in place, but my client wants a more advanced search where you can search via city/country, or the fields of the profile. There are two sections of code where you need to put this advanced search page. One on the main site and the other on the mobile area.
I am completely overworked at the moment and need to hire someone to make this one final page so I can be done with this client. This should not take anymore than 2-5 hours.
This site is built with bootstrap, PHP, and MYSQL.
