CSV/Excel export required to map a template

We have an import csv template that we need to populate with our shipping data. Our export csv contains the data in one row, but the import file splits these fields in to two rows.
I have attached the import file as an example. We need to be able to use the file every day, so it must be easy to import the source data to map it to the correct format.
