Basic Client Database using PHP

I need a client database created so that a client can come to the web site, log in with a username and password so that the system can recall that persons previous information from the last time he filled out this application form. The form will auto fill based on this information and then they user can change whatever he needs to. The submit button currently send this information vis a SSL to anther database. The submit button will also need to take this same data and send it to the database that you are creating.
The form will have aprox. 50 fields of information.
