Applet that upload data to a portal

I need an applet able to:
1) from smartphone or tablet take a picture
2) the picture must be georeferenced using the data available from the GPS/positioning system present in the smartphone/table
3) it must be possible to send the picture, the position, the telephone number of the sender (mandatory), the name of the sender (optional) and a comment (optional) to a portal where an application is running
I need as well an application, based on Drupal (preferred) or Wordpress that:
4) take the data uploaded and automatically
a- properly display on a map (open street map) a flag that open the picture, the time of the uploading, the comment if available
b - put in a db table containing records with the following field: link to the picture, time of the uploading, position coordinates, telephone numer, name of the sender (if present), comment (if present).
The couple applet-application must be integrated and working.
The applet must be ready to pass the approval process of the Google Play store.
