コンテスト概要

I would like a web form where you select the number of tickets and for each ticket I need to collect name, address, age and email address. Once the details are entered the user must accept terms and conditions before paying using paypal, when paypal is successful an email is to be sent to the buyer and the form details are stored in MySQL database. I then need to export a list of all the details in MYSQL to excel. My hosting platform has php and MySQL.
