UPDATE: I HAVE UPLOADED A NEW/SHORTER BIOGRAPHY.

I need a 2-sided 5"x7" vertical flyer that will be used for marketing for a business. I am a Pain Management Doctor starting a new office. I deal with mainly spine pain (neck and back) but also with all joints, headaches, total body pain.

The FRONT: should be eye-catching, inviting, high-resolution image.
1. There should also be a tagline on the front that states: DO YOU HAVE CHRONIC PAIN?
2. It should have my name and credentials easily visible: David J. Kim, MD, MS
3. Should also have my position: Comprehensive Pain Management

The BACK will have my biography (attached in word file) and the contact info for the office (attached in word file).

In terms of design, I would like something bright and modern. For colors I prefer primary colors like blues, white, reds instead of greens/orange. I like cool background graphic design images like hexagons. The flyer should look really trendy and cool and modern. The design should also include my company logo but it should not be the center point of attention. Also need my picture on the flyer and my name next to it again. My name should be very visible: David J. Kim, MD, MS
