I need a website that includes the following: 1. A questionnaire with text boxes and likert responses (with text labels that are linked to numerical values). 2. A report generated for each individual that completes the questionnaire. This report will have text, bar graphs, and spider graphs, which will all be based on the individuals' scores on the questionnaire. 3. The individual should be able to download this report and email it. 4. If they consent for their responses to be used in research, their responses should also be saved to a data file that can be accessed by me. I will also need the R code, which should be all commented so that I can make any desired changes in the future.
