Wordpress - script to output list of pages with parent - > child hierarchy by menu_order and title

We have a plugin that needs a selectize box populated with the title of every wordpress page that exists. We have this output working but now need every parent page to show in order with each respective child page under it. The pages should sort by menu_order then title. See attached script.
