Word / PDF Conversion - FPDF / with a form

I would like to take a 2 page word document and convert it to PDF. However we need to have fields within the pdf which are auto filled in, and a logo image embedded in the pdf file on the fly (top right). In real terms it could be viewed as an application form. Where based on your answers to certain form questions the fields on the application formn pdf are pre filled in for you so that all you need to do is print out and sign it. It is a two page application form with 5 fields on it. The first page contains your responses. The second page is terms and conditions. We will need a) Word to PDF conversion. It must be coded by hand using fpdf methods not embedded as an image (except for the logo). b) Sample form with 6 questions c) Based on your response to these questions the pdf file is created on the fly for you to download. So the task is a) Form with 6 questions - create b) join the form response with the pdf template c) generate the pdf file d) present it for download / save it to file e) store responses in a mysql db (including downloadable form) Notes on coding practices. a) Connection strings must be in an include file b) The form page must be template driven, ie include file for header /footer c) The code must be well formed php using the include files.
