Form to Google Spreadsheet - PHP

Have 10 form fields, you will have to create the html form( no validation needed ) .Once the form is submitted data has to be inserted as a new record in existing google spreadsheet.
The PHP script should check if the record already exists, if yes update it else insert a new record.
Programmer should use their own google account to check. The Script should work on my server.
