I use excel to link all of my forms per project and I want to add the PA notification form to my system. I want the 5 page form in excel format (Excel 2007 Excel Macro enabled workbook), with page 1 at the top and the other four pages below in a vertical column. I would like the check boxes as simple one click check boxes, and the all the other boxes as cells that I can populate from other forms (I will do the formulas but I want you to create the spacing, logos, and formats so that it prints nicely.
