apache with php 5.3.1 on digital ocean droplet

Hi, I need droplet that can run php app that requires 1. Apache with mod_rewrite enabled 2. set subdomain DNS records (so that i can just point the subdomain IP to droplet ip and make it accessable by subdomain/domain) 3. php version 5.3.1 (which can connect to mysql v4) MySQL is not required. the app will be connected to other server running mysql. I need digital ocean image that can be transfered to my account and list of commands you used to install everything, so i can do this myself on another droplet if needed. OS is not important. it can be Ubundu CentOS or whatever.
