Add Contact Form with Calendar and Select Menus to Divi 3.0 Theme

Hello, I need a contact form with select menus and a calendar to be added in place of my current contact box using Divi 3.0. I would like the background color to be the same color as the current contact box (#d1d1d1) as well as the font color #000000 in the fields. Font family must be the same as entire page please. This contact form is off to the right of the page.
