Experienced Web Developer Needed For Ecommerce Product Page, Shopping Cart & Email Customization

I'm in need of hiring an experienced Web Developer that has knowledge in customizing shopping carts, product pages and outgoing/incoming email confirmations. We're a prescription sports eyewear company that sells sports sunglasses & goggles with prescriptions.

Currently, our checkout page does not show what the customer added to their shopping cart or their prescription information. We also want to add a "Notes" section for customers to add to their order on the checkout page.
If you go to any product and click "yes" for prescription, we want to add a prescription form right above the "Try before you buy" and "Add to Cart" buttons. You can view the attachment to this post which shows a mock-up of what we need. After that form gets built in to every product page, we'll want to simply delete this page:

We've also been experiencing issues with our email confirmations. Our customers are receiving email confirmations after they purchase a product but our employees aren't receiving an email copy. The only email we receive is from Authorize.net. We would like to build a completely new email confirmation system and ditch the previous one. This new email system needs to send outgoing email confirmations when a customer places an order and send our employees a copy as well.
Lastly, if you click on "My Cart" in the top left portion of the site. We'll need the "Similar Products" with the text underneath it, deleted entirely. This is located at the bottom of the page.
Overview of what needs to be fixed:
1. Add shopping cart products/prescription to final checkout page where credit card information is entered and adding a "Notes" section.
2. Add a prescription form to every product page as a product type.
3. Fixing our email confirmation.
4. Deleting 'Similar Products' on the shopping cart page.
