Create 2 survey forms on a Wordpress site, send the results to an admin with PGP

We’re looking for someone to create two forms for a small Wordpress site that’s currently under construction. We’ll be choosing a theme and creating the text. It will probably be done with “Parabola” or “Tempera” theme. The site is small, simple, and professional. We ONLY need work creating the two form/survey pages as follows: The form will be on https. I already have the certificate. The completed form is encrypted with PGP and emailed to an admin. The admin uses a PGP enabled email client like Claws and decrypts the data. The admin’s public key is already available. The form will be capturing medical information, and therefore needs to impossible (difficult) for a hacker to gain access to the stored or emailed surveys, and if they get them, the data should be encrypted. People complete the forms on https Anyone with url can fill out survey, it won’t be by invitation or unique URL Data from forms is emailed as comma/tab delimited or excel, encrypted with PGP Any data entered by consumers on the server is encrypted Captcha (or something like it) and form spam protection is required. The form data can be stored or not stored on the server; it really doesn’t matter, since it’s encrypted. I’m aware that the admin will need to install something like Claws, Kleopatra, etc. The survey forms will very standard, capturing the basic name/address info plus about 12 fields, including check boxes, radio buttons, short text, and one comment field. No dropdowns. The form must be responsive. The two forms MUST allow for end-user edits. Ideally, the form will be made with an existing plug-in. All work must be completed in one week from the start date.
