Create a custom captive portal for hotspot

I need a custom hotspot login page for mikrotik router with the ability for the users to self register and login. The progject must use twitter bootstrap and be fully responsive for all devices. It should work with freeradius 3.

The logic is:

When the user connects to the wifi hotspot he/she is presentet with a registration form.

The registration form will create a user based on the details provided and will be automatically logged in to the hotspot without further login procedure.

There are some extra details that will be provided via the chat.
