Create a WordPress Template

Hope all is going well with you. I have a technical question about a site I am building in WordPress. What I am needing is a plugin or some sort of program that will: 1. Allow a person to fill out a JOIN US application form with custom fields that we set, such as name, address, school system, etc.. They will need to be able to log back in with their user name and PW that was created by them when registering, to update the demographic information if they ever change schools, job title, etc. at a later date. 2. This form will need to have fields that when a certain criteria is selected, for example, if it had School System, and they selected Jefferson County School System, it would then only show all the schools in the Jefferson County School System. We will need several fields to allow this type of function. 3. Also, some fields when selected would need to be visible (popup)only when a certain option is selected, otherwise those options stay hidden. For example, if a person chooses, YES, I was a previous member, then a box when appear asking what years and any other information we list in the popup box. 4. All the information that a person provides into our WordPress registration form would need to be downloadable to Excel so we can import the information into a database that we had built using Access. 5. We need to be able to add and delete any of the preset information, such as School System names and School Names, etc. for when new schools are built, etc. 6. This program will need Auto E-mail that say welcome, here is your info you provided, etc, and also send us an email showing that someone has filled out the application/changed demographics, etc.. This can be a prepackaged program or custom built but it has to allow us to be able to control the information once it is installed.
