Appointment booking system

Hi No Wordpress should be used I am looking to have a booking system that visually look like: [login to view URL] I will need to provide like some extra modification and customization : 1- Both RTL+LTR support 2- No registration required. 3- User select the available time and enter his details 4- Adding more fields from text to multiple choice. 5- Admin can add a number of filed such as allowing user to upload documents. 6- Custom time slots for vacation/closed dates. 7- A powerful backend interface to manage appointments. 8- Customizable emails sent to you and your customers 9- Front-end and admin are both fully responsive 10- Fully translatable 11- Required fields Same feature as [login to view URL] But: 1- There is no need for user to register. 2- There should be more fields that can also accept documents to get uploaded. 3- No need for pending status, once user filled the required fields then a customized confirmation email will be sent to user.
