Code CSS and HTML page with Ajax and PHP

I have a CSS and jquery overlay that I need coded with ajax and PHP. Please see the attached files. I need all the buttons to send the value in the input tag to the database.
The ratings 1 through 5 need to send the same number and the number 43434334 to the database.
The eliminate button needs to send a -1 and the number 43434334 to the database.
The award button needs to have a second button that appears that says "confirm" and once you click that, it sends a "Y" and the number 43434334 to the database.
The comment button needs to send whatever is in the text box and the number 43434334 to the database and automatically refresh the comment feed and show the message the client sent in it.
The message button needs to display a text box similar to the comment box where the user can send a new message. When they send a new message it needs to send the message and the number 43434334 to the database.
All this has to be done without the page refreshing.
I can pay $20 for it to be done and need it in 24 hours.
I have all the CSS and styling done and only need to the php and ajax done.
