I'm looking for an application that will generate a certificate of achievement that can be viewed/downloaded and printed from a web browser. This will be used for participants in an online continuing education program.
The application would consist of a pre-designed certificate with boilerplate text and graphics, as well as text fields that will be populated by an HTML form which will be filled out by a facilitator. The form will include the participant's name, the title of the course completed, the date completed, and the facilitator's name; these will all be displayed in the certificate. The HTML form should also include a field for the participant's email address, which will not be included in the certificate, but will be used by the application to notify the participant that their certificate is ready to be viewed and printed. The application will take the information from the HTML form and generate a completed certificate, then email the participant with either the certificate itself as an email attachment, or with a link to a URL where the certificate can be viewed/downloaded and printed.
The application should include a log file to keep track of the generated certificates.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
