10767 Music Code Generator - Simple

Looking for a VERY Simple music code generator..What I would like is a sort of forum like thing..I would put the song name and a checkbox next to it, but obivously a list of them, example "Beyonce - Do it" CHECKBOX. When they select a checkbox, they will hit a generate, submit something button, and in return will receive a code back. The code has to be specific. The generator will in return, have to copy the url that the checkbox specfices and paste it into a line of codes. I know the code, and all that needs to be changed from song to song is the url to file. I Do not want something like not a generator, but they check the box, and the code is pre programmed in. I want a generator. So somehow, with the first selection where they pick the song from the list, that song has the url coded into it and in return generates the code. Thanks guys
